A medical assessment is obligatory for those applying for private hire licence or hackney carriage licence via D4 form.

The medical assessment is carried out by one of our GMC registered doctors.
​
Depending on the council you will be applying to, the doctor measure your blood pressure, perform an eye test and ask you a seris of questions about your health as per your council medical form.

Please bring a copy your medical records from your GP with you for your appointment.

Apply for a medical assessment if:

when you first apply to become a taxi driver

every 5 years when you turn 45, until you're 65 years old

every year after you're 65 years old

What happens during the assessment
The doctor carrying out the examination will ask you a series of questions about your health including your past medical history, alcohol, and smoking history. The medical examination consists of a vision assessment and a blood pressure check. The appointment usually lasts 10-15 minutes; however, this depends on your medical history.

and time of the appointment, please read these carefully.

Your driving glasses and your glasses prescription if you wear glasses (not contact lenses) to meet the eyesight standard needed for driving
