Bus/Minibus

In order to get your provisional Public Service Vehicle licence you will need to to get a D4 form filled in by a doctor to ensure you meet the DVLA Group 2 medical standards and then you will need to send it off with your D2 application form in order to get your provisional licence. 

 

Public Service Vehicles include passenger service vehicle (PSV) / passenger Carrying Vehicle (PCV) over 8 passenger seats (D1, D1+E, D or D+E).

What should I bring to the appointment

  1. D4 form

  2. Photo ID – Driving Licence/Passport

  3. Your driving glasses and your glasses prescription if you wear glasses (not contact lenses) to meet the eyesight standard needed for driving

  4. Your medication prescription

  5. A copy of your medical record from your GP
